Job Overview

At Abe Brown Ministries, Inc., we believe in second chances and the power of restoration. We are currently seeking a compassionate and dedicated Re-Entry Program Case Manager to support clients as they reintegrate into society.

This role is ideal for a human services professional who’s passionate about community engagement, case management, and helping individuals navigate the complex journey of reentry. You’ll work hands-on with clients, families, and community partners to provide support and accountability while upholding the dignity of every person we serve.

Duties and Responsibilities

  • Conduct comprehensive assessments to understand client needs, strengths, and goals.

  • Develop individualized care plans and monitor client progress toward self-sufficiency.

  • Provide consistent case management services through one-on-one sessions, follow-ups, and referrals.

  • Engage in community outreach to build partnerships with housing, employment, and healthcare providers.

  • Document all client interactions, services, and outcomes using the ETO (Efforts to Outcomes) system.

  • Facilitate group sessions, workshops, and support meetings as needed.

  • Maintain strict confidentiality and professional boundaries in all client matters.

  • Participate in team meetings and contribute to program development and improvement initiatives.

  • Transport clients as needed and support them in navigating social services and legal obligations.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred).

  • Minimum of 2 years experience in case management, reentry programs, or human services.

  • Proven ability in client assessment, care planning, and motivational engagement.

  • Strong verbal and written communication skills for effective documentation and client support.

  • Proficiency in basic computer applications and willingness to learn new platforms such as ETO.

  • Demonstrated cultural competence and ability to work with diverse populations.

  • Legal ability to operate a motor vehicle and valid driver’s license required.

  • Commitment to maintaining confidentiality and ethical case management practices.

Salary and Benefits

  • Hourly wage: $20.00 – $22.00 per hour, based on experience.
