Job Overview
21st Century Home Health Services Inc. is seeking an experienced and highly motivated Physical Therapist Case Manager, Level 2 to join our growing team in Folsom, CA. This field-based position focuses on delivering quality home health services to patients across the region, while also providing supervisory oversight to Physical Therapy Assistants (PTAs).

Duties and Responsibilities

Provide physical therapy assessments and treatment plans for home health patients

Supervise, mentor, and support Physical Therapy Assistants (PTAs) in the field

Collaborate with other healthcare professionals to create and update individualized care plans

Ensure accurate and timely documentation in electronic health records

Monitor patient progress and adjust therapy strategies accordingly

Maintain compliance with state and federal home health regulations

Educate patients and families on recovery and mobility improvement techniques

Participate in interdisciplinary case conferences and ongoing team collaboration

Travel to patient homes across the Folsom area and surrounding communities
Qualifications

Valid and unrestricted California Physical Therapist License

Bachelor’s, Master’s, or Doctoral degree in Physical Therapy
-
Minimum 1 year of experience supervising PTAs (preferred)

Current BLS/CPR Certification

Excellent interpersonal, communication, and leadership skills

Strong case management and critical thinking abilities

Ability to work both independently and collaboratively

Familiarity with local community health resources

Understanding of state and federal home health care regulations

Competency in using electronic charting tools and planning software
Salary and Benefits

Base Salary: $125,000 – $160,000 per year
