We are currently seeking a Physical Therapist Case Manager to join our team serving the Santa Clara County territory, specifically around Gilroy, CA. As a Physical Therapist Case Manager, you will play a crucial role in assessing, planning, and managing the rehabilitation care of patients recovering at home. Your clinical expertise, leadership, and problem-solving abilities will directly impact the lives of those we serve.
Duties and Responsibilities

Conduct in-home physical therapy evaluations and develop individualized care plans.

Monitor and adjust patient goals and interventions based on progress and reassessments.

Coordinate with physicians, nurses, social workers, and other allied professionals to ensure seamless patient care.

Educate patients and caregivers on therapy plans, exercises, and expected outcomes.

Maintain accurate and timely documentation using electronic charting systems.

Ensure all services provided comply with federal, state, and agency regulations.

Participate in case conferences and team meetings to ensure continuity of care.

Serve as a clinical leader and resource for physical therapy best practices within the team.
Qualifications

Valid California Physical Therapist License (required).

CPR/BLS Certification (required).

Degree in Physical Therapy.

3+ years of experience in a clinical or home health setting preferred.

Strong organizational and time-management skills.

Proficiency in electronic documentation and patient record systems.

Demonstrated ability in critical thinking and clinical decision-making.

Knowledge of home health regulatory guidelines is a plus.

Excellent communication and interpersonal skills.

Valid driver’s license and ability to travel for home visits across Santa Clara County.
