Duties and Responsibilities
- Assist adult oncology patients as they navigate through the healthcare system, providing seamless coordination of care.
- Conduct initial discussions with newly diagnosed patients (and their families) to determine their needs and identify potential barriers to care.
- Network effectively with physicians and other healthcare providers to ensure the effective implementation of patient care plans.
- Act as a crucial liaison between the family and the inter-disciplinary team, educating and assisting them with understanding the plan of care.
- Provide comprehensive support to patients regarding various issues and concerns, including insurance, transportation, childcare, financial resources, language barriers, and education.
- Initiate referrals within Beacon Medical Group and other community agencies to connect patients with appropriate resources.
- Collaborate effectively with community agencies to coordinate referrals, ensuring patients receive holistic support.
- Utilize interdisciplinary care conferences to plan and coordinate interventions for patients.
- Encourage patient compliance with medical instructions and referrals by providing clear education and support.
- Report information regarding variance trends to the manager/director or appropriate Physician reviewer.
- Coordinate the gathering and reporting of patient outcome information, including follow-ups after procedures.
- Develop and discuss comprehensive survivorship plans with eligible patients and families, outlining treatment received, potential late/long-term effects, and follow-up guidelines.
- Conduct health screenings and provide educational offerings in the community to promote cancer awareness and prevention.
- Maintain an active role in the Oncology Care Committee, contributing to quality initiatives.
- Compile and analyze data for reports, providing insights into patient outcomes and program effectiveness.
- Prepare summaries and reports for the Oncology Care Committee, presenting key findings.
- Manage patients under individual oncology clinics and/or BMG (Beacon Medical Group) clinic, ensuring coordinated care.
- Provide direct nursing care for multidisciplinary clinics as needed.
- Coordinate the scheduling of necessary procedures and follow-up appointments for patients.
- Utilize analytic cases from the Cancer Registry for developing individualized survivorship care plans.
- Deliver and discuss survivorship care plans with eligible patients and their families.
- Attend and participate in department meetings, complete mandatory education/competencies, annual health requirements, and maintain licensure/certification (including BCLS/CPR).
- Consistently utilize appropriate universal precautions, protective equipment, and ergonomic techniques for safety.
- Adhere to all regulatory agency requirements, survey processes, and compliance standards.
- Comply with established organization and department policies and procedures.
- Be available to work overtime and additional shifts/schedules when required to meet patient needs.
Qualifications
- Experience Level: Mid-Level to Senior-Level (Minimum 5 years of oncology nursing experience preferred, but prior leadership experience required).
- Education Requirement: Completion of an accredited nursing program (RN license required).
- Required Skills:
- Completion of an accredited nursing program.
- Current license to practice as an RN in Indiana.
- Oncology Certified Nursing (OCN) certification required (if not certified, within two years of hire).
- Minimum of five years of oncology nursing experience (preferred).
- Prior experience in a leadership role (which utilized problem-solving and conflict management processes) in a healthcare setting (required).
- Thorough knowledge of oncology clinical care practices, procedures, and techniques.
- Thorough knowledge of community agencies, services, programs, and resources for patients/families.
- Knowledge of counseling techniques and ability to identify/utilize appropriate resources.
- Communication skills (verbal, written, listening) to effectively counsel patients/families.
- Interpersonal skills to promote and maintain cooperative, courteous, and sincere relationships.
- Sensitivity and tact when working with patients/families.
- Time management and problem-solving skills, ability to independently prioritize and organize work, ability to work effectively under pressure.
- Computer skills and knowledge to effectively use and support various computer programs.

Your salary will be negotiable and determined based on your extensive experience, qualifications, and leadership background. A $5,000.00 Sign On Bonus is also mentioned.
